New Patient Information

Thank you for taking your time to prepare for your upcoming visit. If you are not already familiar with NEOCS, please look at the rest of our web site that has information regarding our mission, our practice, our cardiologists and Nurse Practitioners/Physician Assistant

Most often our patients are referred to us by their primary care physician (PCP).
Because we are a specialty practice, Insurances often require a referral. It is your responsibility to obtain this referral from your PCP, but we will be happy to assist if necessary.
We normally call 2 days ahead to remind you of your appointment.
It is very important you keep your appointments and if you have to cancel, to give as much advance notice as possible.
If you are a new or returning patient it is helpful to arrive 10-15 minutes before your appointment time.
You will meet with Intake staff who will verify insurance, demographics, review HIPAA and privacy policy and verify who we are able to contact with any messages regarding your care.
It is always a good idea to write down questions for the doctor before you come to your visit.

Bring all of your medication bottles with you.
This will assure we have all your medications listed and the correct dosages. NEOCS will also provide you with refill prescriptions for your cardiac medications at the time of your office visit.

Before you see your cardiologist, NP/PA-C

  • A Medical Assistant will take your weight
  • You will be escorted to an exam room where your vital signs will be taken
  • A family member is welcome to accompany you
  • Most likely a nurse will review your medications and ask several questions regarding your recent health and reason for your visit
  • An EKG may be done prior to seeing the doctor
  • For the EKG it is very helpful if you don’t wear panty hose, use lotions or wear tops that are not easily removed.
  • The cardiologist will listen to your heart and lungs and perhaps do some physical exam
  • Previous tests may be reviewed and a recommended plan of action is discussed with you
  • If tests, procedures or new medications ordered, the benefits and drawbacks will be discussed with you at this time
  • Your information is recorded in our secure electronic medical system (EMR)
  • Before leaving the exam room, the physician, Nurse Practitioner or Physician Assistant will place your orders in the computer
  • At checkout, your co-pay is collected (if applicable), tests scheduled, lab slips given to you, and your next appointment scheduled
  • A receipt is printed for you detailing the visit, charges, and your future appointment
